Bring all forms to our registration/fundraising meeting for all families, both new and returning on January 28^th, 6 p.m., at the Comfort Inn. For the meeting, please bring your player’s:

1. Birth Certificate
2. Player Indemnification form (a notary will be available at the meeting)
3. MHSA physical exam form
4. $50 tryout check
5. The drug policy is currently under review—a revised policy will be released by the meeting date, and can be signed before tryouts
